Appointments for professional eye services

are available Monday thru Saturday.

You may schedule an appointment by calling  (513) 621-0979 for
an appointment Downtown, or (513) 451-6100 for an appointment in
Western Hills.

For new and returning patients, please fill out the
 Patient Information Form linked below
and bring it with you to your appointment.

Note: One office is open each Saturday.  Please call before stopping in on those days to be sure which office is open.

Insurance Plans - We are participating providers with Anthem, Cigna, Humana, Medicare, United Healthcare, and Vision Service Plan.  Many other insurance plans are also accepted.  If your plan is not listed, please phone one of our offices to confirm acceptance.

We respect your time and make every effort to remain on schedule. Please bear in mind that emergencies and unexpected events do occur. If you are unable to keep your appointment, please notify us as soon as possible. We would appreciate at least a 24 hours notice if you must change an appointment.
